Mediator Release Test (MRT): Why Food Sensitivities Matter + How I Use MRT in an FDN “Test, Don’t Guess” Plan

One of the most common “hidden” drivers I see in practice is food sensitivity–driven inflammation—and it can be sneaky because the reaction isn’t always immediate.

This is one reason I sometimes use the Mediator Release Test (MRT) in a root-cause plan.

What MRT is (in plain English)

MRT (Mediator Release Test) is a blood test designed to identify which foods may be triggering an inflammatory immune response in your body. The test evaluates how your immune cells respond when exposed to different foods and food chemicals—and whether that response is associated with the release of inflammatory “mediators.”

In simple terms: it can help us identify foods that may be quietly contributing to your symptom load.

Food sensitivity vs allergy vs intolerance (quick clarity)

Food sensitivities aren’t the same as:

  • Food allergies (often immediate and IgE-mediated—think hives, swelling, anaphylaxis)
  • Food intolerances (like lactose intolerance—often digestive enzyme-related)

Food sensitivities can be delayed and non-obvious, which is why so many people struggle to connect the dots. You might react 24–72 hours later and never suspect that “healthy” food you eat daily.

Here’s what that can look like in real life: you have your usual almond-milk smoothie Tuesday morning. By Thursday, your knees ache and your skin is flaring—and it never occurs to you to connect it back to Tuesday’s breakfast. That’s exactly why sensitivities are so easy to miss on your own, no matter how carefully you’re paying attention.

Symptoms that can show up when sensitivities are part of the picture

Not everyone has the same pattern, but some common ones include:

  • Bloating, constipation/diarrhea, reflux
  • Headaches or migraines
  • Joint or muscle aches
  • Skin flares (acne, eczema, rashes)

I like MRT because it can:

  1. Reduce the guessing (especially when elimination diets feel overwhelming or random)
  2. Lower the immune/inflammatory load so your body can calm down and actually respond to the rest of your plan
  3. Give us a structured starting place for meals (instead of constant restriction and anxiety)
  4. Make progress easier to track (energy, digestion, pain, sleep, skin, cravings, etc.)
  5. Work without an elimination period first—unlike some other food sensitivity tests, MRT doesn’t require you to already be avoiding suspect foods to get an accurate read

How I use MRT in the FDN approach (“test, don’t guess”)

FDN (if you’re new here, see what an FDN actually does) is about investigating—not guessing. MRT isn’t the only tool, but it can be a powerful one when symptoms suggest the immune system is “on high alert.”

Here’s how it typically fits:

  1. Foundations first: protein, blood sugar stability, hydration, sleep, stress support
  2. Build an initial low-reactivity meal plan based on MRT results
  3. Support the body using the D.R.E.S.S. framework:
    • Diet: nourishing, anti-inflammatory, stable blood sugar
    • Rest: sleep, circadian rhythm, recovery
    • Exercise: movement that supports metabolism and resilience (not punishment)
    • Stress reduction: nervous system regulation (because gut + immune function are stress-sensitive)
    • Supplementation: targeted support when appropriate and personalized
  4. Reintroduce foods strategically once inflammation is lower and the body is more resilient

MRT is a starting point—not a life sentence

One of the biggest fears people have is: “Am I never going to eat normal again?”

I want to say this clearly: MRT results are information, not a forever punishment list.

It helps us decide:

  • What to remove for now
  • What to keep for now
  • What to reintroduce later in a structured, confident way
